Timeliness may influence emotional distress during the diagnostic phase of suspected lung cancer patients. We performed a prospective observational study to compare distress and quality of life (QoL) in two medical centres with a Rapid Outpatient Diagnostic Program (RODP) and two using conventional Stepwise Diagnostic Approach (SDA) on the basis of trained nurse-led care.
Purpose: This study evaluates how patterns of psychosocial referral of patients with elevated distress differ in a ‘systematic screening for distress’ condition versus a ‘usual practice’ condition in ambulatory oncology practice.
Methods: The psychosocial referral process in a 2-week usual practice (N= 278) condition was compared with a 2-week ‘using the Distress Barometer as a screening instrument’ (N= 304) condition in an outpatient clinic with seven consulting oncologists.

Mensen wensen in toenemende mate te participeren in keuzes rond de eigen gezondheid. Bij medische keuzes over behandeling of vroegdetectie zijn er vaak verschillende gelijkwaardige opties voorhanden. In dit geval wordt de beste keuze voor een zorggebruiker mee bepaald door zijn persoonlijke waarden en voorkeuren. Bij het maken van ‘de beste keuze’ kunnen beslissingshulpen of “decision aids” helpend zijn. Ze bevatten informatie over de positieve en negatieve aspecten van de verschillende beschikbare opties en helpen de gebruiker om zijn waarden en voorkeuren te benoemen.
Implementation of guidance on assessment and management of psychosocial and supportive-care problems or needs will be successful only if consideration is given to existing skills, experience and expectations of staff and patients. This study examines the roles and responsibilities of staff, patients and families in relation to management of social difficulties and proposes a pathway for response.
Social difficulties may add to the psychological burden experienced by cancer patients. Therefore identifying social difficulties in routine oncology practice may help prevent or alleviate distress. The Social Difficulties Inventory (SDI) is a short questionnaire developed for assessing social difficulties in cancer patients. Although well-validated, not enough is known about the clinical meaning and utility of the instrument or whether the items can be meaningfully summed to form a summary index of ‘‘Social Distress’’.

The International Psycho-Oncology Society (IPOS) has championed the need for quality care to incorporate attention to the psychosocial concerns of cancer patients. Widespread international endorsement of distress as the ‘6th vital sign’ is a major step towards improving access to psychosocial care and reducing the isolation and stigma experienced by many affected by cancer.
